Rollin San Juan yesterday eve, and my foot got caught on the inner part of the teal before the switchbacks, my foot canted on the pedal got caught on the dirt wall and twisted my knee 90 degrees.

I'm in pain now, can barely walk.

Have any of you had a similar injury??

If I go to the E/R what's the best they could do??

Neverthless you'll have one less rider in the trail this week because of this..;(

That sucks man, I haven't had any knee injuries yet, but I'm assuming they will give you something to help with the pain and refer you to a specialist. they may take xrays but Im not sure if they can actually do anything to help out.

My suggestion is know exactly where the pain is so that you can get the as close to the problem as possible, Ie the Knee behind the joint, The knee just below the joint.

Hopefully you just stretched it the wrong way too far and you don't need any surgery or PT!

Thanks, the pain is mostly around the inner part of the knee near the connector muscles of the hammy and quad. It sucks, to not be able to ride in a beautiful day like this. I'm looking to see if I tore my meniscus. Or Saint Juan tore my meniscus!!

It's either the PCL or the ACl depending on which way the foot twisted; if you did not feel and hear a significant "POP" then you probably only strained either of these ligaments. Keep it elevated above the level of your heart, ice, ice, and more ice, and lots of Motrin ( or another NSAID, but not Tylenol)- stay hydrated. As far as the meniscus is concerned, it's more of a minor issue within the soft tissue of the knee joint, I wouldn't worry about that as much. If you are able to pedal (like on a stationary bike) that will help with blood flow to the injured area and help dissipate the swelling- but only if it's not that painful to do it. Try to keep your range of motion increasing by NOT leaving your leg straight all day long, keep it moving.

As far as the ER is concerened, you'll be there for HOURS, they will call for the orthopedic surgeon on call, and he will take his time coming in on a weekend since it's not a compound "open" fracture. It's what they would call a "soft tissue" injury. If you have insurance, just rest for today, and call your doctor tomorrow for a consultation with an ortho doc. They will order an MRI and only with that will they be able to see what and how bad the damage is.

Agree with RR, don't bother with the ER, it will be a waste of time and money especially if you have an HMO. Wrap it to try to keep the swelling down and ice it frequently. Go see your doc if it's not feeling better within a few days. I'm not a doc so take my advice with a grain of salt, I have had a few knee injuries though!

If you can get an ortho referral maybe check out Orthopaedic Specialty institute and see Dr. Peng. He's there foot and ankle guy. I twisted my ankle pretty good last year and he fixed it right up.

It sounds like you may have torn your MCL. I've done it before surfing and it is very painful. The bright side is that it usually heals on its own without needing surgery. Another good thing is that the best rehab for that injury, once you get decent mobility back, is riding your bike.
