that's a classic. i think ross made the apollo 3 speed. now that was a sweet rig!!!! banana seat, ape hanger bars. those are some huge a$$ reflectors as well

There is nothing wrong with brake lever extenders if they are designed right. I have them on my 20 year old Panasonic road bike and will not buy a new bike until I can get one with brake extenders.

Some high end road bikes now come with them. The design is more complicated and more expensive but they have the same basic functionality.
